Semaglutide vs Tirzepatide: Key Differences to Discuss With a Clinician

A practical, non-prescriptive comparison of semaglutide and tirzepatide for weight-management research.

Reviewed August 19, 2026 · Independent editorial content

Semaglutide and tirzepatide are different active ingredients with different mechanisms and FDA-approved brand indications. A clinician should determine whether either is appropriate for an individual patient.

Mechanism

Semaglutide is a GLP-1 receptor agonist. Tirzepatide activates both GIP and GLP-1 receptors. Those pharmacologic differences are one reason they should not be treated as interchangeable products.

Brand versus compounded forms

FDA-approved products containing these active ingredients exist, but compounded preparations are a separate regulatory category. FDA does not approve compounded drugs before marketing for safety, effectiveness or quality.

Cost and access

Cash-pay telehealth pricing can differ substantially. HealthRx currently advertises a lower starting monthly equivalent for compounded semaglutide than compounded tirzepatide.

What to ask your clinician

Decision principle

Do not choose solely because one drug is cheaper or because a provider advertises greater average weight loss. Individual appropriateness, safety, product status, access and follow-up all matter.
